- 2010 Player Medical Release Form with Notary (FL State regulation, required of all U.S. teams)
- 2010 Player Medical Release Form without Notary (Foreign Teams only)*
- Hotel Form
*Note that without a notarized medical release form it is possible that medical attention may be denied by emergency personnel or hospitals for any injured player. USA Volleyball is not responsible for any denied medical care or claims.
Additional Required Forms (Non-USA Teams Only)
- 2010 Foreign Team (FT) Summary Form (1 per Team)
- 2010 Foreign Team (FT) Registration Form (1 for each individual on the roster-staff and players)
*Special Note for all Canadian Teams: You must send a letter of good standing from Volleyball Canada with your team entry and check.
